A written signed statement that the medication is "For Personal Use Only". This statement is required for Customs clearing. It can be written and signed by the physician, patient or caretaker. A copy of a photo ID (driver's license, passport etc.) is also recommended to ease Customs clearing. Payment can be made with a personal check -made payable to "October Health Inc"-, a bank Money Order or with Master Card/VISA/AMEX credit card (please include the expiration date). Cheques for orders over $300.00 must be made certified by your financial institution. Send information via regular mail to mailing address or, for faster service, please fax all information to 1866-557-8446. Orders are shipped within 72 hours of the receipt of the documentation. Delivery is approximately 10 to 12 business days for Domperidone and Parsitan.  Other specialty medicines may take up to 21 days for delivery.
